    Triagem robusta de melanoma : em defesa dos descritores aprimorados de nível médio

Considerando que a presença de um dermatologista em tempo integral não é economicamente viável para muitas cidades e especialmente em comunidades carentes, ferramentas de auxílio ao diagnóstico para a triagem do melanoma têm sido um tópico de pesquisa ativo. Muitos trabalhos existentes são baseados no modelo Bag-of-Visual-Words (BoVW), combinando descritores de cor e textura. No entanto, o modelo BoVW vem se aprimorando e hoje existem várias extensões que levam a melhores taxas de acerto em tarefas gerais de classificação de imagens. Estes modelos avançados ainda não foram explorados para rastreio de melanoma, motivando assim este trabalho. Aqui nós apresentamos uma nova abordagem para rastreio de melanoma baseado nos descritores BossaNova, que são estado-da-arte, mostrando resultados muito promissores, com uma AUC de 93,7%. Este trabalho também propõe uma nova estratégia de pooling espacial especialmente desenhada para rastreio de melanoma. Outra contribuição dessa pesquisa é o uso inédito do BossaNova na classificação de melanoma. Isso abre oportunidades de exploração deste descritor em outros contextos médicosAbstract: Melanoma is the type of skin cancer that most leads to death, even being the most curable, if detected early. Since the presence of a full time dermatologist is not economical feasible for many small cities and specially in underserved communities, computer-aided diagnosis for melanoma screening has been a topic of active research. Much of the existing art is based on the Bag-of-Visual-Words (BoVW) model, combining color and texture descriptors. However, the BoVW model has been improving and nowadays there are several extensions that perform better classification rates in general image classification tasks. These enhanced models were not explored yet for melanoma screening, thus motivating our work. Here we present a new approach for melanoma screening, based upon the state-of-the-art BossaNova descriptors, showing very promising results for screening, reaching an AUC of up to 93.7%. This work also proposes a new spatial pooling strategy specially designed for melanoma screening. Other contribution of this research is the unprecedented use of BossaNova in melanoma classification.     Knowledge Transfer for Melanoma Screening with Deep Learning

    Knowledge transfer impacts the performance of deep learning -- the state of the art for image classification tasks, including automated melanoma screening. Deep learning's greed for large amounts of training data poses a challenge for medical tasks, which we can alleviate by recycling knowledge from models trained on different tasks, in a scheme called transfer learning. Although much of the best art on automated melanoma screening employs some form of transfer learning, a systematic evaluation was missing. Here we investigate the presence of transfer, from which task the transfer is sourced, and the application of fine tuning (i.e., retraining of the deep learning model after transfer). We also test the impact of picking deeper (and more expensive) models. Our results favor deeper models, pre-trained over ImageNet, with fine-tuning, reaching an AUC of 80.7% and 84.5% for the two skin-lesion datasets evaluated.Comment: 4 page

    GeoSES: A socioeconomic index for health and social research in Brazil.

    The individual's socioeconomic conditions are the most relevant to predict the quality of someone's health. However, such information is not usually found in medical records, making studies in the area difficult. Therefore, it is common to use composite indices that characterize a region socioeconomically, such as the Human Development Index (HDI). The main advantage of the HDI is its understanding and adoption on a global scale. However, its applicability is limited for health studies since its longevity dimension presents mathematical redundancy in regression models. Here we introduce the GeoSES, a composite index that summarizes the main dimensions of the Brazilian socioeconomic context for research purposes. We created the index from the 2010 Brazilian Census, whose variables selection was guided by theoretical references for health studies. The proposed index incorporates seven socioeconomic dimensions: education, mobility, poverty, wealth, income, segregation, and deprivation of resources and services. We developed the GeoSES using Principal Component Analysis and evaluated its construct, content, and applicability. GeoSES is defined at three scales: national (GeoSES-BR), Federative Unit (GeoSES-FU), and intra-municipal (GeoSES-IM). GeoSES-BR dimensions showed a good association with HDI-M (correlation above 0.85). The model with the poverty dimension best explained the relative risk of avoidable cause mortality in Brazil. In the intra-municipal scale, the model with GeoSES-IM was the one that best explained the relative risk of mortality from circulatory system diseases. By applying spatial regressions, we demonstrated that GeoSES shows significant explanatory potential in the studied scales, being a compelling complement for future researches in public health
